What to Bring to Your Appointment
To ensure a smooth process, please bring the following:
- Photographic ID – Preferably a Driving Licence or Passport
- Your GP Surgery Details – Name and address of your registered GP practice
- Your Medical Form – e.g. D4, Taxi form, or any other required form
- Glasses (if worn) – Bring both reading and driving glasses. If you wear contact lenses, please bring a spare set or your glasses, as you will be tested with and without lenses
- Medication List – A list of any medication you take regularly, as well as details of any recent or active hospital appointments and tests
